Project Narrative: Provide a clear and comprehensive project narrative (1000 words max) encompassing realistic project goals, a timeline reflecting planning and implementation of the project, a description of the audience that the project is designed to attract, plans for promoting, publicizing and/or disseminating the project, and identification of performance or exhibition venues if relevant. If the project is based on copyrighted material, include a statement concerning clearance rights. Annual Budget: NOTE: You must provide both income and expense information in your budget Budget - Expenses: List all costs associated with your project; be sure to include costs for all activities listed in your project narrative. These may include fees for artistic and administrative personnel, travel, accommodations, materials, equipment, space renal and and other expenses associated with your specific project. Include budget notes when necessary to help understand how you arrived at the listed amounts (e.g., $10,000 – Artist fees (5 artists @ $500 per week for 4 weeks). Budget Income: List all sources of income for the project, including contributed funds, earned income and in-kind contributions. (Note that in-kind contributions are non-cash contributions of goods and services, and must be reflected on both the expense and income sides of your budget.) Indicate specific sources and amounts of funds, and indicate whether each is committed (already received) or projected (you have apply or intend to apply). Include budget notes when necessary. The income should be equal to the expenses listed in your budget. Total Project Budget: Fundraising Strategy: A 250 word narrative overview of how you intend to raise money for your project, including a fundraising timeline. Describe why the sources of income you have listed in your budget are feasible and appropriate. Principal Staff: Include resumes for principal artistic and/or administrative staff, and indicate each person’s role in the project. Principal staff should demonstrate ability to complete project goals. Work Samples: Please provide samples of previous work or a work-in-progress by the principal artists. You may submit a total of two items: jpeg’s (a maximum of 8), manuscripts, CDs and DVD’s are acceptable. Do not send originals. Work samples should be recent (from the past five years), of high quality and as relevant to the project as possible. Whether you email or mail your work sample, make sure to include a Work Sample Description. This blurb should describe your work sample, and its relevance to your proposed project, in less than 50 words. Letters of Support: Please submit letters of support or interest from key members/aspects of the project such as committed artists, venues, clearance of music rights etc. LETTERS OF SUPPORT ARE NOT MANDATORY but can strengthen an application. |

Organizational Status: Submit one copy of your Article of Incorporation document and one copy of your Federal Employer Identification Number document. If not incorporated, please provide proof, from lawyer or online application receipt, that you are in the process of incorporating. Copies sent via mail will be accepted as well. Applicants submitted without these documents will not be considered. Work Samples: Work Sample (Preferred but not required): You may submit a work sample in support of your application. You may submit a total of two items: brochures, jpegs (maximum of 2), manuscripts, CDs and DVD’s.
